Download the MARAC referral form.

Attached the Dash risk assessment

It would be useful if you could send us a copy of the Dash risk assessment you completed with the victim. If this is not possible, then send us a list of the risks and vulnerabilities you have identified.

Email us the completed form

After you've completed the form, email it to us alongside your Dash risk assessment or list of risks and vulnerabilities.

Email: marac@dumgal.gov.uk

Filling in the referral form

You should tick the correct box in the referral form, based on the risk assessment you conducted with the victim.

High risk (if the score if over 14)

If you think the victim is at considerable risk of domestic violence from their adult partner, ex-partner, or family member.

Professional judgement (if the score is under 14)

If you think the victim is at risk or you have identified risk markers like: previous abuse, sexual jealousy, control, and separation.

Tick 'Escalation'

If there have been three or more Police callouts, or A and E attendances, or housing repairs due to damage in the last 12 months

Tick 'Repeat'

If there has been a previous referral to MARAC (same victim and perpetrator) in the last 12 months and a new incident has occurred or the risk has increased.

After you've made a referral

The MARAC coordinator acknowledges the referral.

You should let your colleagues and the MARAC representative for your agency that a referral has been.

Ideally the professional who has submitted the referral should attend the MARAC meeting and present the case to the MARAC; if this is not possible, please ask a colleagues from your agency or department to present the case in the meeting.

A MARAC meeting is then organised.
